Emergency Evacuation Plan

Warning to Evacuate Project: An Air Horn will be sounded by Consigli personnel. (3 Long Blasts)

All personnel will use the existing stairwells, or any safe means of exit available, for evacuation of the project/building.  Evacuation routes will be updated and communicated as the project evolves.

If the area must be evacuated all personnel shall head to the designated Muster point.

**The evacuation from building will necessitate gathering to a muster point. (see evac plan).**

The foreman for each company will take a count of their staff.  If for some reason a worker is unaccounted for, the fire department will be notified of the last location where the worker was observed.

If there is a fire then the alarm (if operational), or air horn protocol listed above, will be sounded and 911 contacted immediately. If the emergency is a small fire that can be contained with the fire extinguishers provided, then an employee may attempt to extinguish the fire. However, no employee is to jeopardize his or her safety attempting to extinguish a fire.  The building will be evacuated. 

  1. General Conduct and SITE-SPECIFIC requirements
    1. All injuries, no matter how minor, shall be reported to the Consigli Superintendent immediately.   In any event, no injury shall exceed 24 hours from the time the injury occurs until notice is filed.
  2.  Consigli has a strict “Anti-Retaliation” policy regarding the reporting of accidents.  No one has the right to retaliate against employees for reporting work place injuries or illnesses. 
  3. Building operations will take precedence over any construction activity.
  4.  Occupied Areas.  Workers will need express permission from CCC before accessing areas of the building outside of the designated work area.  If you enter areas without Consigli Supervisory permission, you shall be removed from the project immediately.
  5. Traffic Rules, Regulations, and Parking.  Posted speed limit will be followed at all times; pedestrians/students/staff in walkways will always have the right of way.
  6. Any areas or entrance located below elevated work will be barricaded or protected prior to any work commencing.
  7. Any act or threat of physical violence, harassment, intimidation or other threatening disruptive behavior that occurs at the project is grounds for immediate dismissal from the project.
  8. Horseplay will not be tolerated and will be grounds for dismissal.
  9. The building is an active facility and will be occupied during the first stage of construction.  All workers will respect the facility, its staff and visitors at all times. There will be a zero tolerance for any inappropriate behavior.
  10. Workers shall restrict their activities and presence to the areas of their assigned work and shall not enter into at any time, including lunch or relief periods, those areas open to staff and visitor unless authorized to do so.
  11. All personnel shall have, at a minimum, OSHA 10 Hour Construction Safety Outreach Training Certification and shall exhibit documentation of training. 
  12. A Job/Activity Hazard Analysis shall be conducted on each work task to be performed.  The JSA/AHA shall be reviewed initially and then weekly with all personnel performing that particular task.  Signatures of all personnel including supervisors involved will be required.
  13. All CCC personnel performing work with their hands shall be required to wear gloves that are appropriate to the task.  When not performing actual work with their hands workers will be required to have them available for immediate use…   
  14. Loud and profane language as well as clothing with offensive language will not be allowed on the jobsite. Extra efforts must be made to keep noise to a minimum.
  15. The use of any tobacco products on grounds is strictly prohibited.
  16. No radios, i-pods, i-phones, portable CD players or headphones will be worn while on site.
  17. Workers are to only use designated bathroom facilities.
  18. Caution will be used with combustible engines of any type being placed next to the air intakes located on the building.
  19. Normal working Hours will between 7am until 3:30pm Monday through Friday, any overtime will need the approval of the CCC Superintendent with a prior notification of at least 72 hours.
  20. All task which create or are suspect to create a loud noise will be performed in such a manner so as to limit its impact on the occupants.
  21. At no time, shall a worker disturb or move a material he/she can not readily indentify as either asbestos or lead free, as listed in hazardous material reports. They shall stop and notify their supervisor/Consigli project superintendent immediately.
  22. Consigli will be notified no less than 72 hours before any task that will require the need to work outside of the designated project area.
  23. All Foremen will be responsible for coordinating all material deliveries with the Superintendent (at least 48 hours’ notice) and making sure that all vehicles are unloaded without haste, so that unloading time is limited.  (no deliveries before 7am)
  24. Environmental controls must be used to keep dust to a minimum.
  25. All debris from this project will be removed daily. 
  26. All debris from this project to be removed by way of an occupied space shall be done so in covered carts.  Covers, carts, and all associated wheels are too be wiped clean before leaving the construction area.
  27. Any HVAC outlets leading out or into the work area must be covered by an approved filter media.
  28. Any penetrations leading into occupied areas during construction will be fire stopped by the end of the shift.
  29. Workers will seek CCC approval prior to drilling, sawing or any other form of penetration from construction area into the occupied area.
  30.  Environmental controls must be used when possible to keep dust to a minimum.
  31. Any damage caused to the building will not be tolerated or accepted.  Exercise extreme caution when working in or around the buildings so as not to cause unnecessary damage or wear and tear.  If there is a question regarding your work, ask before you cut, hammer, drill, demo etc.  Due to the historic nature of the building, items to remain cannot just be replaced if they are damaged.  Again, exercise EXTREME CAUTION there will be ZERO TOLERANCE for ANY damage caused to the building or its surroundings.
    1. LEAN REQUIREMENTS
  32. No materials shall be delivered to the site earlier than 3 days before said materials are to be installed/put into place.  If materials have to be delivered before that 3-day time period, subcontractor shall get prior approval from Consigli Superintendent.  Because of this, notification / scheduling is required for all deliveries to the site. Subcontractors must notify Contractor at least 24 hours in advance. Contractor has the right to refuse any deliveries not properly scheduled or due to logistical constraints as necessary.
  • All subcontractors delivered materials shall be placed on wheeled carts, wheeled racks or in wheeled bins so as to necessitate easy relocation in the event materials need to be moved.  Carts, racks, or dollies shall not exceed manufacturers intended weight loads.  Materials/tools can be delivered and stored on pallets only if pallet jacks are delivered simultaneously with said delivery to provide for ready mobility.  Additionally, pallet jacks need to remain in immediate area.    Each subcontractor will need to supply their own pallet jacks for their own materials and keep their pallet jack within reasonable distance at all times.
  • All work stations shall be provided with wheeled trash bins for immediate placement of all debris produced as a part of the subcontractor’s operations.  All trash will be immediately placed in wheeled containers provided by Consigli.
  • All subcontractors shall use rubber wheeled carts when moving material or removing trash from a building.  Any damage caused by the Subcontractor shall be repaired at the cost of the Subcontractor.  Back charges will be appropriately assessed for the cost of the repairs.  No trash or materials shall be left on the floor. 
  • Subcontractors shall, where feasible, elevate all electrical extension cords, hoses, or cables – removing them from all walking/working surfaces.  Electrical Cords, when elevated shall be supported or suspended in a manner that does not subject them to damage. 
  • Storage of delivered materials in cardboard containers shall be discouraged.  When materials must stay in cardboard containers, said containers shall be removed from the projects immediately after product is unpacked. 
  • Subcontractor will participate in schedule development meetings as required by Consigli.
    • Substance Abuse
  • As a condition of hire, all CCC personnel are required to submit to a urinalysis test for determination for the presence of illegal substances.  A positive result will disqualify applicant for employment.
  • The use, possession, concealment, transportation, promotion, or sale of any of the following substances will be grounds for disciplinary action, up to and including termination from employment on said project.
  • Alcoholic beverages
  • Marijuana and other illegal drugs, look-a-like, and designer drugs
  • Drug paraphernalia
  • Controlled substances such as medication when usage is abused or when the substance is possessed without proper prescription   labeling
  • The use, possession, concealment, transportation, promotion, or sale of any of the following controlled items will be grounds for disciplinary action, up to and including termination from employment on said project.
  • Firearms, weapons, and ammunition
  • Unauthorized explosives including fireworks
  • Stolen property or contraband
  • We expect all individuals to treat each other with respect. We are committed to maintaining an environment in which employees and visitors are not subjected to different treatments because of legally protected characteristics. Therefore, we will not accept or tolerate any discrimination, different treatment or harassment of or among employees, customers, suppliers, students, faculty or visitors based upon the following characteristics:

    • workplace violence

Workplace violence is any act or threat of physical violence, harassment, intimidation, or other threatening disruptive behavior that occurs at the work site. It ranges from threats and verbal abuse to physical assaults and even homicide. It can affect and involve employees, clients, customers and visitors.

There will be a ZERO tolerance policy in effect for any workplace violence event matching the aforementioned behaviors.

Procedure:

  • Any act or threat of physical violence, harassment, intimidation, or other threatening disruptive behavior that occurs at the project is to be reported to the Consigli Project Superintendent immediately.
  • Call to be made to Consigli Corporate Safety Director and VP of Field Operations
  • Investigation by Consigli Superintendent shall happen immediately
    • Conflicting parties to be separated and placed into 2 separate locations and interviewed by Consigli Superintendent as to facts alleged.
    • Witnesses’ to be identified and statements taken
  • Upon investigation – if physical assault or threatened physical assault took place – Business Agent (if applicable) shall be contacted immediately, individuals removed pending investigation outcome, and, if necessary, contact Police.     
  •  Complete Consigli “Workplace Violence Investigation Document” and forward to Safety Director and VP of Field Operations

Any act or threat of physical violence, harassment, intimidation, or other threatening disruptive behavior that occurs at the work site is grounds for immediate dismissal from the project. 

  1. Each CCC sub contractor is to assure that their respective area is policed and cleaned daily.  There are more than enough trashcans and disposal units so that debris should not be found anywhere on this job site or surrounding areas.
  2. Construction debris is the sole responsibility of each Sub-Contractor. Dumpsters are provided by Consigli for the use of all trades. 
  3. Any Contractor who is unable or fails to remove construction debris generated by their workers on a daily basis will require that Consigli accomplish this task, however, the added cost for this cleanup will unfortunately be borne by the Sub-Contractor.
  4. When it comes to housekeeping Consigli would prefer that we all work as a team and strive together to keep this project clean and safe.
  5. Slips, trips, falls on stairs and falls from elevations are the most common work site injuries. We can stop these injuries by keeping well maintained and clean walking and working areas material will not be stored in stairwells at any time.
  6. All fuel containers will be in appropriately labeled UL rated safety containers. If stored   overnight they must be stored in a lockable rated cabinet which is no closer than 50 feet to the nearest structure.
  7. All cylinders must be marked with the company’s name for easy identification.
  8. All cylinders must be clearly marked indicating their contents.
  9. All cylinders must be stored in an upright position and secured from accidental displacement. No more than a day’s supply shall be stored in any building.
  10. All Oxygen/acetylene set-ups must have the regulators removed and protective caps replaced at the end of the working day. All set-ups shall be kept in hand carts during work hours and separated by 20 feet at the end of the working day.
  11. LPG (Propane gas) cylinders are not permitted to be stored anywhere within the building
  12. When cylinders are not hooked up to an appliance they are considered to be in storage and must be removed from the building.
  13. Cylinders, whether full or empty must be kept in an enclosure outside the building.  This enclosure should be a chain link fence a door that can be locked.  No smoking signs shall be posted on the fence and a fire extinguisher located nearby. Empty and full cylinders should be kept separated within the enclosure.
  14. Any open flame work such as cutting, brazing, soldering, welding, etc., creates an opportunity for a fire.  An internal “Hot Work Permit” shall be obtained from the Consigli Construction Superintendent daily and completed prior to work being performed.  At least one ten-pound ABS fire extinguisher will be kept close at hand for each set-up.  If it is deemed necessary, a fire watch may be necessary.
  15. All hot work must terminate at least one hour before quitting time.
  16. Temporary buildings, when located within another building or structure, shall be of either noncombustible construction or of combustible construction having a fire resistance of not less that 1 hour
  17. Temporary heating devices should not be installed or used by anyone on the job until authorized by the Project Superintendent and cleared with the United States Capitol Police/ local Fire Department.  Electric Space heaters shall not be used.
  18. When tarpaulins or canvas coverings are used on the project, they must be a U.L. listed material made of a fire-resistant material with a flame spread rating of 15 or less. These materials should be securely fastened.
  19. Welding shields shall be utilized for all welding operations in which peripheral workers are exposed. Welding leads shall be free of damage 10’ from work end, also connections will be booted. Repairs to welding leads will be made with non-conductive material/tape (3M rubber Splicing Tape-130C
  20. Welding fumes shall be controlled through ventilation when done indoors.
    1. Cranes and Rigging
  21. All crane operators, riggers and signalman must produce their certification/qualification papers and maintenance records to Consigli’s Superintendent prior to any work taking place.  Inspection records must have been produced by an independent inspector.  All lattice boom and tower cranes shall have an independent 3rd party certification during set up and upon completion of rig assembly.
  22. All rigging and hoisting components must have rated capacities affixed to them and be free of all defects.  Any wire rope slings found to be defective must be removed from the site. 
  23. Only properly licensed operators shall be allowed to operate hoisting equipment.  Oilers and apprentice operators shall not be allowed to perform lifts they are not qualified to perform.
  24. Written Lift plans shall be conducted for all crane work to be conducted.  Said plans shall highlight weight of load, radius of crane operation for said pick, and crane rating (off manufacturers load chart) for said weight and radius configuration.  Any lifts over 75% capacity will require additional planning and will be considered critical in nature. 
  25. All workers must be trained in that specific piece of equipment per OSHA
  26. All workers, when required by individual states, shall produce those licenses to the Consigli.
  27. Operators manual and extinguishers must be with the equipment at all times
  28. Load chart must be posted in the cab at all times
  29. Equipment regardless of weight shall not be allowed to operate directly over the utilities buried on the SW elevation unless weight spreading panels have been previously installed.
    1. , and mast climbers
  30. Tubular Welded Scaffolds are to be erected on base plates and/or mud sills only.
  31. FRAMEWORK – All scaffold components must be of the same manufacturer. Mismatching of components will not be allowed.
  32. GUARDRAILS – All scaffolds shall require fall protection and toe boards when platform height is six feet or more above the any working surface. Workers will be tied off 100% of the time while erecting or dismantling scaffold.
  33. Baker/mobile scaffolding will be required to be equipped with guardrails when the fall exposure is greater than 4 feet or per manufacturer requirements.
  34. Form Bracket scaffolds, Wall Bracket scaffolds will be equipped with guardrails when the fall exposure is ten feet or more.
  35. Rolling and Mobile scaffolding must have all wheels locked prior to any employee accessing the work platform.  Also, scaffolds are not to be moved while workers are on the platform. (No riding on scaffold by workers).  Casters must be in the locked position. Outriggers will be used whenever work platform height to base ratio is greater than two to one.
  36. All scaffold planking shall be scaffold grade.  Dry rot or large cracks deem the planks to be unacceptable and shall be removed accordingly. Unacceptable planking can be used for concrete forms, mud sills or toe boards.  All scaffold platforms are to be clean and clear of debris.  Additionally, all scaffold work platforms/planks shall be kept clear of ice and snow at all times and prior to any one walking or working on said planks or platforms.
  37. A “Competent Person” (one who is able to identify hazards associated with the work he/she is supervising and have the authority to stop work whenever he/she determines that an operation is proceeding in an unsafe manner) must inspect the scaffolding before work begins or if any change is made in the scaffolding.  This review must be documented by initialing the color coded (red, yellow, and green) “Scaffold tag”.  Any deficiencies found shall be brought to the Superintendent immediately. The inspection Guide shall be provided upon request
  38. Aerial lifts require the use of safety harnesses at all times.  All Aerial lifts shall be inspected daily.  This review must be documented and submitted to Consigli’s Superintendent daily or the operation will be stopped until said document is submitted.  All operators shall be trained in the use of the lift that they are using.  This training must be documented and submitted to Consigli’s Superintendent.  All areas below aerial lifts shall be cordoned off to disallow anyone from working or passing below area where overhead work is being conducted.
  39. Anyone working on mast climbers shall be trained in the safe operation and use of those mast climbers prior to entering the mast climbers to perform work.
  40. Any one working on mast climbers must assure their respective competent person has inspected and approved that the mast climber is safe for the work they will be performing.  Said inspection will be required daily and documented in the inspection log accordingly. 
  41. Hardhats:  Hard hats shall be worn at all times while on this job site.
  42. Identification Badging/hardhat emblems:  All CCC workers on this project will be given a hardhat emblem
  43. Clothing:  Work shoes and/or work boots shall be worn at all times. Shorts, sneakers, cut-off T-shirts and tank style shirts are prohibited.  High visibility vests shall be worn at all times when exposed to vehicular traffic of heavy equipment.
  44. Safety Glasses:  Shall be worn at all times.  All Safety glasses shall meet ANSI Z 87 requirement.  Face shields shall be used when there is a potential for facial injury (i.e. grinders and chop saws)
  45. Hearing Protection:  Hearing protection will be worn when appropriate for the work being performed, IE. Saw cutting, operating of powder actuated tools, etc.

6.   All CCC personnel performing work with their hands shall be required to wear gloves that are      appropriate to the task.  When not performing actual work with their hands workers will be required to have them available for immediate use.

  1. Ladders must Type 1A or 1AA, be in good repair with no cracked rungs or rails
  2. Ladders must be tied-off or otherwise secured against accidental displacement
  3. Extension ladders must extend at least three feet above the working surface
  4. Step ladders are not to be used in the closed position
  5. The top two steps of any step ladder are not to be used for standing
  6. Fall protection is required when working on a ladder which is located in a location that has a change in elevation greater than 6 feet or when workers are exposed to a fall greater than the fall from the ladder.
  7. Fall protection systems must be utilized when workers perform activities at or above six feet above the working surface. Fall protection systems include guardrail systems, netting, and personal fall arrest systems (Body harnesses, lanyards, and an anchorage points rated for 5000 pounds). This includes, but is not limited to, steel erection, roofing, scaffold erection and dismantling, overhand bricklaying and precast concrete related work. 
  8. Access to Roof Decks, Loading, and Controlled Access Zones, or any areas that do not have permanently affixed guardrails where a fall exposure exists, shall be controlled through Consigli’s internal fall protection access permitting program managed by the Consigli Superintendent and/or the assigned Site Safety Manager.  The “Fall Protection Access Permit” shall be issued daily for all areas where permanently affixed guardrails are not installed (roofs, etc.), have been removed with approval, or for designated controlled access zones where personal fall arrest systems will be required.
  9. The Consigli “Fall Protection Access Permit shall be completed any time fall protection will be removed for any reason.
  10. Tying off when working in an Articulating Boom Lift is required at all times.
  11. Any fall exposure greater than six feet will require fall protection. Safety Monitors are not allowed.
  12. Elevator pits will require fall protection.
  13. Where a wall opening or edge, whether exterior or interior is protected by the placement of studs and the spacing of the studs does not exceed 12 inches, and if metal studs used are no less than 22 gauge, then no additional fall protection is required.
  14. All areas located below work area shall be cordoned off to prevent any unauthorized access. (Red Danger Tape)
  15. All entrances/emergency egress located directly below any type of work task shall remain free of construction debris and material and be provide with overhead protection.
  16. Floor openings (2 inches or greater) shall be protected by the installation of standard guardrails or covered with materials that will carry any weight that the cover may be exposed to (2x the intended axle load if equipment is present). Covers shall be secured and painted with the words “Hole”.
  17. All contractors are to have First Aid supplies available for their employees.
  18. Any employee injured in the course of their employment on this job site will report the incident to their supervisor immediately. The employee’s supervisor will in turn immediately report the incident to Consigli’s Site Safety Manager or Project Superintendent. 
  19. The supervisor’s accident report, supervisor’s incident investigation report, & employee’s injury report shall be submitted to Consigli’s Site Safety Manager and or Superintendent whenever a Sub-contractor employee is injured. These forms need to be completed by end of the workday of that accident.
  20. Any accident involving damage to property shall be reported to Consigli’s Superintendent immediately.
  21. Any trench greater than 5 feet in depth requires completion of the Consigli Construction “Excavation Checklist”
  22. Any Sub-Contractor involved with excavation work shall have a competent person on site at all times while excavation work is taking place.
  23. A “Competent Person” is one who is able to identify hazards associated with the work he/she is supervising.  Further, a “Competent Person” must have the authority to stop work whenever he/she determines that an operation is proceeding in an unsafe manner.
  24. All soils will be classified as Type “C” unless field-testing conducted by a competent person determines otherwise.
  25. Type “C” soils mean that the walls or banks of a trench must be sloped back at a rate of 1 ½ to 1 once the depth of the excavation exceeds 5 feet.
  26. Ramps or ladders must be located so that an employee is not required to walk more than twenty-five feet before reaching such ladder or ramp.
  27. A “Competent Person” must inspect any excavation that remains open overnight, after any weather event or any time before any work commences after a break in work.
  28. All workers who will be disturbing soil or otherwise performing any task in an excavation will be required to have a Hazwoper 40.

1. All electrical tools, cords and equipment must be in good working order. To that end, all electrical tools and components must be visually checked on a daily basis.  Any damaged tools or cords must be taken out of service until repaired or replaced.

2. All electrical outlets must be of the Ground Fault Circuit Interrupter type (GFCI).  These receptacles should be checked on a daily basis by simply pressing the test button and then the re -set button. The on-site electrician should conduct monthly tests of each GFCI and keep a log of his findings.

3. All cords must be equipped with a ground pin.  Flat three wire cords are not allowed.  SJO and SJT or other similarly rated cords only shall be used.

4. All tools must have a ground pin on the plug unless the tool is manufactured as a double insulated tool.   Double insulated tools have an identifying mark on the case, usually a square within a square.

5. Double insulated tools will be attached to a three-wire grounded cord.  Because a worker has a double insulated tool does not mean he/she can plug into to a cord with a ground pin missing.  This practice would expose other workers not using double insulated tools to the possibility of plugging into a defective cord.

6. No work shall be conducted on live electrical.

7. All temporary lights must be protected on all sides.  All temporary electric will be suspended by nonconductive materials

  1. A confined space is defined as an area which
  2. Is not meant for continuous use.
  3. Has adequate size and configuration for employee entry
  4. Has limited means of access and egress
  5. Is not designed for extended employee occupation
  6. Before any work is commenced in what might be considered a confined space, Consigli’s Superintendent shall be notified.  Consigli’s Superintendent and the Foreman of the company who will have employees working in a confined space will determine the requirements for working in this space.
  7. Before entering a confined space, an atmosphere test of the confined space must be conducted, and the results noted. If any contaminant is found, mechanically blown fresh air is required at all times. The employee entering the space must wear a safety harness with a lanyard. A second person is required outside the space to monitor the worker.  If the person in the space is out of view of the monitor, then a radio may be required.  In some cases, a tripod with a rescue line attached to the worker’s body harness may be required.
  8. Before entry is made into any manhole, duct, vessel or any other confined space, as defined by the OSHA 1926 standard, Consigli Construction’s confined space entry permit shall be completed by the trade contractor’s supervisor/competent person and a copy shall be submitted to Consigli’s Superintendent.
    1. /Global harmonization
  9. All Contractors’ on site are required to submit a copy of their HAZCOM Program and a company safety manual to Consigli’s Superintendent.
  10. All HAZCOM programs will be kept in the field trailer for everyone’s convenience.
  11. Any time a Contractor brings a new product onto this site they will insure an SDS for this material is placed in their program.
  12. All HAZCOM Programs will have an index of SDS Sheets to facilitate finding the sheet relating to the product being checked.
  13. SDS Sheets for products on site will be kept in each Contractor’s book. 
  14. Any questions regarding products should be first addressed with the affected employee’s supervisor. However, no employee will be denied the opportunity to look up an SDS for any product he/she is required to use or a product another contractor is using or is about to use.
  15. SDS Sheets alert employers and employees to any precautions that may be required when using a particular product/material
    1. Environmental exposures – lead, Asbestos, and Pcb’s
  16. The paint on all painted surfaces will be presumed to contain lead paint unless otherwise noted in Haz Mat report which will be located in the field office and available for review.
  17. All subcontractors will be required to perform exposure assessments for any planned tasks on painted surfaces identified as having lead paint…
  18. Any work on lead paint related building components (stairway rails) will require workers to have lead awareness training before any work is done.
  19. Consigli workers may need to perform work on the original Building surfaces.  They may need to disturb this paint by drilling, sawing, demo, fastening, or carpentry.  In doing so they will use saws, drills, screw guns, impact wrenches, sawzalls, hand wrenches crowbars, hammers, and other hand tools
  20. Consigli construction does possess historical data to show that drilling, sawing, light demo, installation of hangers, hanging sheetrock, fastening and carpentry do not put the employee above the action level.  Despite this, additional monitoring may be done if needed.
  21. Consigli or subcontractor workers performing activities other than those listed above must be monitored to insure they are not being exposed to a hazard.
  22. Consigli employees will be provided with tyvek suits and gloves at their request.
  23. If paint flakes or dust is generated from lead paint, then the area must be cleaned using HEPA vacuums or wet washing using TSP.  Areas should not be dry swept.  They may be swept using a sweeping compound as long as lead dust is not made airborne and other methods are infeasible.
  24. Workers who contact potential lead paint or other contaminated surfaces should use proper hand and face washing procedures prior to eating, drinking, smoking, chewing tobacco, chewing gum or applying cosmetics.  A hand wash station will be established and maintained for the project duration.
  25. Subcontractor foremen are responsible for reviewing hazmat reports with their employees and ensuring their compliance with the safety rules. All workers on this project must have completed Lead awareness training prior to starting on this project.

    1. Asbestos

Hazardous Materials report has been prepared by the building owner is located in the field office and is available for review at any time.

  • Asbestos has been found in the planned work areas and will be abated. This does not mean that they do not exist elsewhere on the site. Workers will stop and notify CCC if they discover any material they cannot readily identify as asbestos free … The complete table and “List of Materials tested, Quantities, and Locations” can be found with the hazardous material report.
  •  This list and complete Hazardous Materials Report will be available on site all times.   At no time, shall a worker disturb any material he/she can not readily identify. 

All asbestos abatement will be preformed by a licensed Asbestos removal/abatement contractor.

Any asbestos removal will be done under the supervision of qualified personnel.

The asbestos removal plan (if needed) and Haz Mat report will be available for review.  It is the responsibility of all subcontractor foremen to share this report with their respective workers.

All removal will be completed, prior to any one accessing said areas. At no time, shall a worker disturb or move a material he/she can not readily identify, if they discover such a material they must notify the Superintendent immediately. All workers on this project must have completed Asbestos awareness training prior to starting on this project.

  1. PCB’s (POLYCHLORINATED biphenyl)
  2.  PCB’s are not suspect in any of our planned work surfaces/task. At no time, shall a worker disturb any material he/she can not readily identify.  If they come across a material he/she can not confirm as asbestos free, they shall notify their supervisor immediately. If PCB’s are found in existing finishes.   All suspect PCB containing finishes which test for more than 50 ppm it will be considered PCB containing material and will be disposed of in accordance with all local, state and federal regulations.
  3. Light ballast that are not labeled “NO PCB’s” will be considered PCB containing, All PCB containing material shall be removed and disposed of in accordance with all local, state (MEMEP Special waste management rules 06-096-CMR 400) and federal regulations and be coordinated Harvard.

  5. At no time, shall a worker disturb or move a material he/she can not readily identify, if they discover such a material they must notify the Superintendent immediately.
    1. Mercury

All fluorescent lamps are presumed to contain mercury vapor, which is a hazardous substance. Mercury-containing lamps associated with fluorescent light fixtures in areas designated to be renovated will be treated accordingly.  All florescent bulbs removed during renovation will be collected and be recycled or disposed of properly.

  1. Freon/Glycol

All Freon/Glycol materials will be removed from equipment and stored per manufacturer’s recommendations before it is disturbed.  All workers who will be working with the above-mentioned materials must provide CCC with certifications or qualifications.  If not re-used, a disposal manifest will be provided to CCC.

  1. Silica

Any activity which can produce silica dust will use engineering controls to keep dust to a minimum.  A written exposure plan must be developed for all silica producing tasks that clearly defines the following:

 1) A description of the tasks in the workplace that involve exposure to respirable crystalline silica.

2) A description of the engineering controls, work practices, and respiratory protection used to li60 State St employee exposure to respirable crystalline silica for each task.

 3) A description of the housekeeping measures used to limit employee exposure to respirable crystalline silica.

 4) A description of the procedures used to restrict access to work areas, when necessary, to minimize the number of employees exposed to respirable crystalline silica and their level of exposure, including exposures generated by other employers.

 There will be no dry cutting of any masonry material allowed; if water cannot be used then a dustless vacuum system consisting of HEPA dustless vacuum system will be used.  Prior to the project going full scale an exposure assessment of a worker grinding and cutting a silica containing material while using the HEPA system an exposure assessment will be done so as to establish the silica exposure level. 

  1. Miscellanous Safety issues;
  2. At no time, should workers be anywhere outside of the work areas during normal working hours.  Break and lunch areas need to be coordinated with the CCC Superintendent. 
  • Safety related violations will be documented and managed by issuing a formal “safety violation notice” (to be forwarded to company office as well).  In the event of a second offense, with the same individual, a second violation notice will be issued, and the individual shall be removed from the project permanently. CCC reserves the right to dismiss anyone at any time.
